- Be sure you have an unexpired government-issued identification with your picture. The ID will need to be in English. This includes Drivers License, Passport, State-Issued ID Cards, or Military IDs. If I come to you, you'll need to have a clean, well lit area, and a cleared table and chairs in which to handle the paperwork. If this isn't feasible, we can meet at a library or restaurant.
Kentucky (KRS 423.325) allows Notarial Officers to accept the oath of two credible witnesses as to the signer's identity. The credible witness must have an unexpired, government-issued picture ID with their signature.
I will never notarize a document if the individual appears to have cognitive impairments. However, if the impairment is physical, Kentucky law allows for Signature by Proxy (KRS 423.335). This would require two witnesses one of whom will be the signer on your behalf. Witnesses used must be unaffected by the outcome of the document. I can provide these witnesses for an additional fee.
An unexpired government-issued identification with your picture. The ID will need to be in English. This includes Drivers License, Passport, State-Issued ID Cards, or Military IDs. If adequate identification isn't available, a Credible Witness can be used. (See the FAQ section: "What if I don't have an ID?")
Kentucky law prohibits me from offering legal advice or drafting legal documents. These issues need to be handled by an attorney.
